When coming to receive the H1N1 vaccine, each employee will be asked to complete a:
Consent form from the St. Louis County Health Department
Washington University Screening Tool.
The employee ID number on the screening tool must be completed for tracking purposes back to the Department of Health. Your Washington University ID badge is also required to receive a vaccine.
If anyone is unable to come to this session, they will be able to attend future vaccination sessions as they are scheduled and communicated. You can also check the http://www.wustl.edu/flu web site for other University related flu information.
